  3. If the claim is paid through EFT or Credit Card and the payment is not cleared and paid date has crossed 7 days then what will be your next question?
    Provide the correct check mailing address to the rep & ask to reissue a new check
    What is the reason?
    Ask to reissue a new payment
    Could you please fax the EOB? If not then mail it or provide the source to get the EOB
    which one is correct
