Medicaid - QMB, SLMB, QI & QDWI Program

1) These are the Medicare programs established to help low-income beneficiaries where Medicaid pays their Medicare Part A & Part B premium, Medicare coinsurance, and Medicare Deductible.

2)Below are the Medicare programs and their covered benefit,
  • QMB (Qualified Medicare Beneficiary) Program: Medicaid will pay Medicare Part A premium, Medicare Part B premium, Medicare coinsurance, deductible & co-payment.
  • SLMB (Specified Low-Income Medicare Beneficiary) Program: Medicaid will pay Medicare Part B premium.
  • QI (Qualifying Individual) Program: Medicaid will pay Medicare Part B premium.
  • QDWI (Qualified Disabled & Working Individual) Program: Medicaid will pay Medicare Part A premium.

3) You will find a scenario where a claim was paid by Medicare as primary and Medicaid denied the claim as secondary stating non-covered benefit, so you can check Medicaid eligibility to verify the patient enrolled program and covered benefit.

4) For example, When you check Medicaid eligibility & patient has an SLMB plan and the medicare left balance is coinsurance then Medicaid will not cover it since SLMB covers Medicare Part B premium only then the claim needs to be released to the patient. (Note: In general guidelines, Medicaid patients should not be billed for any amount but check your client update and work accordingly. If Medicaid patient billing is allowed then you can release the claim to the patient. If it is not allowed then the balance needs to be written off.)

  1. Can you check on point no 2 SLMP states Medicaid will pay Medicare Part B premium,but point no 4 Medicaid will not covered it since SLMB covers Medicare Part B premium only.This is something messed up in understanding.please clarify.

    1. Just consider the same example which is mentioned above on point number 4 but when patient has QMB plan then medicaid will be responsible for left over coinsurance amount.

  3. How should we apply QMB programme if we are already enrolled in Medicare .

    1. To enroll in QMB program, you must be entitled to Medicare Part A and your income must be below the poverty level which is defined by your state.

      If you meet both of the above conditions then you can enroll in QMB program.

  4. primary insurance wellcare (medicare hmo) gave PR2, And secondary insurance medicaid denied as co96 and patinet enrolled in QMB PLAN? HOW TO WORK

    1. Please check EOB if there is any remark available. If not then make a call to medicaid for more details. This amount must be covered by Medicaid unless there is another reason for denial.

  6. May I know if medicaid cover 100% than why it make MCO?

    1. It is not necessary that Medicaid will always pay 100% except for pregnant women and children. The Guidelines differ from state to state and Medicaid may charge for out-of-pocket or other charges based on the individual's condition.

      The requirement of MCO is occurred because Medicaid covers a very large number of people now and through MCO, it is able to cover a good portion of Medicaid members.
