Medicaid Insurance Policy ID Format for All States of United States

  • State Medicaid insurance must be billed based on the patient's state but in some cases, the patient has multiple addresses of different states and if the claims are billed to Medicaid insurance with the policy ID of any other Medicaid then it would be denied for invalid policy ID or patient not identified.
  • In such cases, policy ID would be helpful to identify the correct state Medicaid when the correct address is not available.
  • Below is the list of state Medicaid with the correct policy ID format and examples,

  1. Update Connecticut Medicaid ID format.. Actually for CT Medicaid thier ID format is fully numeral with 9 digits..

    And there are a dozen state that don't have a format listed. Do you have data on these?

    1. Apologies for the delayed response. Actually, it took time to gather policy ID of the pending state medicaid. We are able to find the policy ID of most of the state medicaid but still pending for 3 states - ME, RI & PR. We will update those once find in the future and medicaid policy ID of all other states have been updated in the above list.
