United Healthcare Plans with correct Payer ID & Policy ID Format to avoid Denials

  • When working on UHC plans, you would come across a scenario where the patient would be active on dos but still claim is rejected by the clearing house or the claim is not on file by the payer or received a denial as the claim is not covered by this payer.
  • This could happen because the claim was billed to the incorrect payer ID.
  • A payer ID is a unique ID that is assigned to each insurance company. This ID is used for submitting claims electronically through our system.
  • Below are some plans of UHC with their policy ID format and payer ID, on the basis of the policy ID format you can find out the correct payer with the correct payer ID to bill the claim to the right payer.
  • You can also verify the correct payer by checking the eligibility on the web portal before resubmitting to insurance.
  • For example, if the patient has a policy ID of 8 digits number and the claim was billed to UHC then from the above list, you can identify that the policy ID of 8 digits number belongs to UMR. You can check the eligibility for UMR with the same 8 digits number then you will definitely find the patient details on UMR. Then take the action accordingly.
  • AARP with a policy ID of 11 digits number is always secondary to Medicare. So, if the claim is not already processed by Medicare then obtain the correct policy ID for medicare and bill it.
