Important Note:
  • When the referring provider is not enrolled with Insurance or Group then ask the client to provide the next action or get the enrollment completed.
  • When the referring provider specialty is different then ask the client for the correct referring provider details or further action,
  • If other referring providers' details are not available then adjust the claim.
  • In the CMS 1500 form, the referring provider's name and NPI will be available in box# 17 and 17b respectively.
  • When referring provider information is available in CMS 1500 and the rep is still not able to find it from their end then resubmits the claim again, information may be missing due to some error.
  • When referring provider information is not available on the claim form then resubmit the claim to check whether information still going on the claim form or not. If it is still missing then inform the same to client.

    1. There is a possibility that the provider is not allowed to perform the denied CPT. You may check the payment history if the same provider has received the payment for the same CPT previously then you can ask the insurance rep to reprocess the claim after providing paid claim details.

      If the provider has never received payment for the same CPT then confirm the reason with the insurance rep.
