Important Note:
  • If the rep does not have primary insurance details then checked in the system if there is any other insurance available or patient's payment history has any other insurance as primary, if yes then check eligibility for that insurance and resubmit the claim to that payer if the policy is active as primary or else release the claim to the patient if the policy is inactive or no other insurance information available.
  • You can also check the payer web portal to get primary insurance details if access is available.
  • When the rep provides the primary insurance information and you have web portal access for primary insurance then always verify eligibility through the website, there could be a possibility that primary insurance is inactive on dos then ask insurance to reprocess the claim.
  • When the rep provides all details of primary insurance then you can update that insurance as primary and make current insurance secondary insurance and resubmit the claim to primary insurance.
  • If the claim is already paid by primary insurance then Click Here to follow primary EOB scenario.

  1. I have a question can anyone please help me?
When we check the eligibility of Medicare insurance it shows that UHC is primary insurance for the patient and we billed the UHC insurance as primary, Primary paid the claim and remaining balance stated as Patient responsibility. Now when we billed the Medicare as secondary insurance for remaining balance, Medicare denied the claim as claim not covered by this payer you must send the claim to correct payer. In this scenario what should we have to do can we you release the bill to patient or not?
Please suggest what's the best way to resolve this issue.

    1. If you check the medicare eligibility and find the UHC payer under MCO or HMO option then the claim cannot be billed to Medicare as secondary. UHC insurance processes the claim as primary on behalf of Medicare and if there is no other insurance available after the processing of UHC then you can bill the claim to the patient.

    2. Hi, for the procedure code A0425 A0428 whom we need to bill, as they are Transport charges, as I have only one payer well care, it denied for misrouted claim, so we bill to NEMT but they dont have any portal to check claim status, so I called them but they said claim not found, facing same issue in all the same kind of claim

    3. Hi,

Could you please confirm the following information?

When Wellcare denied the claim as a misrouted claim, did you reach out to a Wellcare representative to clarify the reason for the denial? The insurance representative may provide the correct details where you can bill the claim.
